asked the Minister for Health if he will arrange to have a long term illness book issued to a person (details supplied) in Dublin 16 in view of the seriousness of the case.

A completed application form for a long term illness booklet in respect of the person concerned was received quite recently by the health board. Upon examination of this case health board officials considered that a medical card would be more appropriate in view of the particular nature of the case involved. Accordingly, a doctor's acceptance form was forwarded to the person concerned on 6 February 1986 and the completed form was received by the board on 13 February 1986. A medical card for the person concerned is currently being processed and will be issued in the near future. I should state that once the doctor's acceptance form is signed the person concerned is entitled to receive all medical card benefits.
