Question:

189 Mr. G. Mitchell asked the Minister for Health and Children the reason a person (details supplied) in Dublin 12 cannot get an application form for a medical card signed by a general practitioner. [17685/01]

I understand from inquiries made by my officials to the South-Western Area Health Board of the Eastern Regional Health Authority that the applicant in question has now obtained the required general practitioner signature.
