View answerI wish to confirm that reports have been sought and received on the matter from both the Irish Prison Service and the Garda Síochána. It would appear from examining the reports that there was a breakdown in communications in conveying the fact that the individual concerned had been charged with new offences. The parties concerned are reviewing their procedures to avoid any recurrence of this type of situation.
