The person named above submitted an application for a Companion Free Travel Pass and her application was desk assessed by a Medical Assessor on 13 May 2013. A medical report was completed by the person’s medical practitioner and this was scanned by the Department. A Medical Assessor viewed the medical report in line with evidence based protocols and Departmental guidelines in order to determine medical eligibility for the scheme. Following the desk assessed medical assessment, the medical opinion was conveyed to Household Benefits Section and a decision letter issued to the applicant on 14th May 2013.
