View answerI would like in the first instance to offer my sympathies to the family involved in this very unfortunate case. I am advised that previous communications earlier this year to my Department from the family in question sought a Coroner's Inquest into this sad death. The decision to hold an Inquest is a matter for the Coroner. The Coroner is entirely independent in his functions under the Coroner's Act 1962 and the family were advised of the position. I have referred this case to the HSE and asked them for a report on the matter. I will revert further to the Deputy in this regard.
