In regard to meeting the particular group referenced by the Deputy, I can advise the him that am acutely aware that any proposed changes to health services can be a source of great anxiety and worry for communities affected, and I am determined that any changes to the service at Our Lady’s Hospital Navan must be done in consultation with the local community.

I have therefore instructed the HSE to pause in the planning of these changes in order to commence an engagement process on the plans, including with elected representatives on behalf of local people. The Deputy will be aware that this engagement is underway and I look forward to engaging with him in that regard.

Civil service payscales are approved by the Department of Public Expenditure and Reform (DPER) in line with the public sector pay agreement, Building Momentum.

Building Momentum provided for an increase to certain civil service payscales on 01 October 2021 and this was applied by DPER to all relevant payscales on that date. All civil service payscales will increase on 01 February 2022 by 1% under the Sectoral Bargaining provisions (Chapter 2) of the agreement.

Reflecting this adjustment, the Department of Public Expenditure and Reform has instructed my Department that the rate for the post of Secretary General in the Department of Health with effect from 01 February 2022 is €297,869 (PPC).

The immunisation programme in Ireland is based on the advice of the National Immunisation Advisory Committee (NIAC). The Committee's recommendations are based on the prevalence of the relevant disease in Ireland and international best practices in relation to immunisation. It makes recommendations on vaccination policy to my Department.The HSE has been advised to operationalise updated NIAC guidance in relation to the mixing of vaccines, specifically relating to certain people who are yet to complete a two-dose Covid-19 vaccine regimen.The NIAC advice on heterologous vaccination relates to:Individuals who have already had a first dose of Vaxzevria® (AstraZeneca) and who did not complete the vaccination schedule as recommended and have requested an mRNA second dose. If an mRNA vaccine is used as a second dose, it should be given after an interval of at least 28 days and the person should be considered fully vaccinated.Those with a contraindication to one mRNA COVID-19 vaccine should not receive another authorised mRNA vaccine. Consideration may be given to non-mRNA vaccination for anyone 18 and older including pregnant women. This should be given after an interval of at least 28 days and the person should be considered fully vaccinated.It is important to note that the number of people with an absolute contraindication to Covid-19 vaccination is very small. There are very few people who cannot receive one of the current vaccines due to pre-existing allergies or history of anaphylaxis.My Department is considering the issues arising for those people that cannot receive a Covid-19 vaccine for medical reasons. The Deputy will be aware that on 21 January the Government announced the removal of most public health measures. From 22 January, the requirement to have a Digital COVID Certificate to enter various premises in Ireland has been removed. Following a Prime Time programme in which the nursing homes referenced by the Deputy were featured, I convened a meeting with the Chief Inspector of the Health Information and Quality Authority (HIQA). In the context of that meeting, I requested a briefing report, to include the current position regarding those nursing homes. This was prepared by HIQA and was submitted to me in July 2021. The report was based on the findings of HIQA’s inspection reports on the three centres.

The Chief Inspector, HIQA, reports on their inspection findings and judgments in an inspection report. In the context of the three nursing homes in question, HIQA has confirmed that the inspection reports on which the briefing note was based have all been published on HIQA’s website in line with the normal regulatory process.

Deputy Francis Noel Duffy asked the Minister for Health if his attention has been drawn to the fact that Tallaght University Hospital has discouraged the public from attending the emergency department for less severe casualties and to consider alternative care options due to long waits and pressure on the department; if his Department has consulted with the HSE on surge capacity plans and other measures to ensure that the hospital has the capacity and resources needed to meet the number of patients entering the emergency department; and if he will make a statement on the matter. The HSE Winter Preparedness Plan for Winter 21/22 builds on the significant investment in last year’s Winter Plan and the additional capacity delivered in health services in 2021, and centres on three core objectives: ED avoidance, patient flow and hospital egress to mitigate the expected challenges in providing emergency care this winter while also continuing to respond to Covid-19.The Plan recognises that a whole system response is required and outlines how the HSE proposes to manage these challenges across primary, community and acute care including measures to allow the public system to access private healthcare capacity.This plan endorses a home first approach, enabling and facilitating patients to receive the most appropriate care in their own homes and communities.The Plan contains a number of initiatives to deliver its core objectives of ED avoidance by treating people at home or in the community and by allowing them to return home following hospitalisation quickly. These initiatives include:

- GP Liaison Nurses to manage direct referrals from GPs to ED;

- Geriatric Community Support;

- Enhancement and expansion of Frailty Intervention Therapy Team models;

- Community Response Teams (nursing and therapies);

- Community respiratory admission avoidance teams;

- Expansion of the National Ambulance Service Pathfinder initiative

- Additional home support hours and

- COPD outreach teams.

Emergency Departments across the country are experiencing high attendance levels. The EDs must prioritise those with serious or life threatening injuries and illnesses. There are other options available to those requiring less urgent care. For example, many patients with limb injuries can go to an Injury Unit and others with illnesses needing less urgent treatment can be seen and treated by their GP or referred by their GP to an Assessment Unit the following day.

In relation to the particular query raised regarding the emergency department at Tallaght University Hospital, as this is a service matter, I have asked the Health Service Executive to respond to the deputy directly, as soon as possible.

The Regional Health Areas (RHA) Advisory Group has held two meetings. The first meeting was an introductory meeting held on the 21st of December 2021, where the members were given an opportunity to introduce themselves to the group and review the terms of reference.

The second meeting took place on the 24th of January 2022. At this first substantive meeting, the group discussed the vision for RHAs and considerations for clinical and corporate governance in RHAs.

The Department is currently in the process of creating a dedicated webpage for the RHA Advisory Group where the notes of the meetings will be uploaded and available for the public to review. We plan to have this in operation no later than next Friday the 11th of February. Department officials will inform the Deputy of the address of this webpage when it is live to facilitate convenient review of all RHA Advisory Group meeting notes.
