In 2012 it was agreed at the time with the Acute Coronary Syndromes Programme that a confirmed STEMI (ie diagnosed in the field by a paramedic/advanced paramedic) should be transported to the nearest Primary PCI Centre within 90 minutes of diagnosis, bypassing other hospitals. Since then, this protocol has not changed.

The National Heart Programme (NHP) is the overarching body charged with development of acute and community cardiovascular care on a par with international standards. It has specific briefs to ensure equity of access to hospital and community care structures is integral to the development of care pathways that support the implementation of Sláintecare.

The National Review of Specialist Cardiac Services is examining the delivery of cardiac services nationally with the aim of ensuring that as many patients as possible have access, on a 24/7 basis to safe, quality assured, and sustainable emergency interventions following an acute myocardial infarction.  The Review seeks to achieve optimal patient outcomes at population level with particular emphasis on the safety, quality, and sustainability of the services that patients receive by establishing an optimal configuration of a national adult cardiac service.  The review engages with a range of stakeholders as part of this process.

National Maternity Hospital

As the Deputy will be aware, on 17 May last, the Government approved the legal framework that will underpin the relocation of the National Maternity Hospital (NMH) to the St Vincent’s campus at Elm Park.

In terms of next steps in relation to the legal framework documents, the Co-ordination Agreement between the three signatories – the HSE, the NMH and St Vincent's Healthcare Group - sets out the practical arrangements for finalisation of the documents. I understand that the HSE will be engaging with the NMH and SVHG with a view to completing the signing and execution of this document in the short term. The remaining legal framework documents will subsequently be signed at the relevant time as the project progresses. 

I have asked the HSE to respond to the Deputy directly in relation to the detail of next steps, as soon as possible. 

National Maternity Hospital

The Government approved the legal framework that will underpin the relocation of the National Maternity Hospital (NMH) to the St Vincent’s campus at Elm Park, on 17 May last.

In terms of next steps in relation to the legal framework documents, I understand that the HSE will be engaging with the NMH and SVHG with a view to completing the signing and execution of the Co-ordination Agreement in the short term. I am advised that this document sets out the practical arrangements for finalisation of the legal framework documents. The remaining legal framework documents will subsequently be signed at the relevant time as the project progresses. 

Separate to the legal framework, a Final Business Case for the relocation of NMH has been submitted to my Department by the NMH Project Board for review at Gate 2 of the Public Spending Code (PSC). The Business Case is now subject to technical review by the Department of Health, including through the External Assurance Process recently introduced by the Department of Public Expenditure and Reform.

Pending favourable review under the PSC, a Memorandum for Government will then be brought forward to progress the programme for the NMH. If approval in principle is granted by Government, the project can move to deployment of a tendering strategy.

Based on the results of public procurement process, an updated Final Business Case will then be prepared by the NMH Project Board, seeking Government sanction to proceed with the main capital project to construct the new NMH. The final decision to proceed with the construction and therefore to estimate the cost or key dates for the new NMH cannot be made until the tender process has been completed and the costings reviewed to ensure that the proposal delivers value for money and remains affordable.
