All payments due have now been made in this case.
Completion of a prescribed form of declaration is a requirement for the payment of pension or allowance. In this case a form of declaration was issued to the person referred to in the Deputy's question with the notification of the award of widow's allowance. As the completed form was not received in my Department, payment of the allowance had to be withheld.
