570 Mr. Yates asked the Minister for Health and Children if persons (details supplied) in County Wexford will be furnished with all the individual patient immunisation records for their son relating to the three in one vaccine; the reason for the delay in the health board issuing these to the family and the lack of communication with the family arising out of ministerial and direct correspondence with the health board; and if all the requested documentation will be made available to them. I have made inquiries of the South-Eastern Health Board in this case. The health board does not hold comprehensive records of batch and lot numbers of vaccine used during the period of this child's vaccination, which predates the establishment of the South-Eastern Health Board. Searches in the Wexford and Waterford areas in 1998 recovered some records indicating lot numbers and batch numbers. However, in neither area was any record found of lot numbers from batch 3741 of Trivax vaccine.

In order to assist the family in this case the health board continued to make investigations in order to try and locate any information or records which might relate to the individual concerned. The health board has been unable to locate any immunisation records relating to this individual. However copies of some reports and records relating to a hospital stay have been located.

The South-Eastern Health Board regrets any apparent delay in contacting the family. The board has assured me that it has recently provided the family with the information it has traced in relation to their son.
