Dental Services Provision

Deputy Seán Crowe asked the Minister for Health if his attention has been drawn to the affect the recruitment embargo is having on the provision of public dental services for children in the Kildare West Wicklow area; the measures he is taking to address this issue; and if he will make a statement on the matter. The Government has decided that the numbers employed across the public service must be reduced in order to meet its fiscal and budgetary targets. The health sector must make its contribution to that reduction. However, the HSE can make staff appointments once it remains within its overall employment ceiling and has the financial resources to do so. As the provision of public dental services is a matter for the HSE in the first instance, the Deputy's enquiry has been referred to the Health Service Executive for direct reply.

Hospital Services

141. Deputy Joe McHugh asked the Minister for Health if he will update Dáil Éireann on his Department's review of proposals to establish a North-South deep brain stimulation service; and if he will make a statement on the matter. [30021/13]

As stated to this House on 16 April 2013, the health technology assessment conducted by the Health Information and Quality Authority and published in 2012 found that the cost of a national deep brain stimulation (DBS) programme in Ireland would exceed the cost of continuing to fund DBS through the Treatment Abroad Scheme (TAS) by approximately €21,000 per patient over 10 years. Over five years, the incremental budget impact of a national DBS service would be €1.84 million more than the estimated €4.29 million required to treat the same number of patients through the TAS.

No formal proposal has been received from the Health Service Executive in relation to the development of a cross border DBS service. However, mindful of the potential of cross border collaboration to improve the quality of service and value for money, officials from my Department are making initial enquiries of their Northern counterparts as to the potential for the development of a collaborative service in this area.

Hospice Services

142. Deputy Aengus Ó Snodaigh asked the Minister for Health the additional support that will be provided to the three existing children’s hospitals pending the completion of the new hospital at the St. James’s site, Dublin; and if he will make a statement on the matter. [30395/13]

The three existing paediatric hospitals in Dublin - Children's University Hospital Temple Street, Our Lady's Children's Hospital Crumlin and the paediatric service at Tallaght - will come together in the new children's hospital when built. The HSE National Service Plan 2013 sets out the quantum and type of health services to be provided in 2013 to the three hospitals, within the overall level of funding provided.

The three hospitals have stated their commitment to becoming a single service across three sites and are working closely together, with a single Clinical Director across the three sites, to ensure optimisation of resources and facilities.

On 6 November last, I announced that it was my intention to establish a Children's Hospital Group Board and in April I appointed Dr Jim Browne to chair that non-statutory Board. This is an important step in moving towards a single entity and in supporting the operational integration of the three existing hospitals in advance of the new hospital being completed.

Substance Misuse

Deputy Mary Lou McDonald asked the Minister for Health the reason the drugs advisory group has not met since January; if it is to be replaced by a new structure; when this new structure will be in place and fully functioning; if he will outline the terms of reference and details of this new structure; and if he will make a statement on the matter. The Deputy will be aware that I announced details of a series of measures last December arising from a review of Drugs Task Forces in order to better equip them to respond to the current pattern of substance misuse. These measures included the extension of the remit of the Task Forces to include alcohol addiction and the establishment of a National Coordinating Committee for Drug and Alcohol Task Forces. This new committee will replace the Drugs Advisory Group.

As measures to address the abuse of alcohol remain under consideration by the Government, I am not yet in a position to provide clear time-scales for the establishment of the new committee. Broadly speaking, the committee will drive implementation of the strategies on substance misuse at local and regional level and will oversee, monitor and support the work of the Task Forces.

Home Help Issues

Deputy Clare Daly asked the Minister for Health the reason he has not yet honoured the Labour Court recommendations which ordered the payment of a gratuity of four and a half weeks for home helps who were employed in the sector between 2000 and 2008; and if he will make a statement on the matter. The employers of the home helps concerned are funded by the HSE under Section 39 of the Health Act 2004, which provides that the Executive may assist a body that provides a service similar or ancillary to those which the Executive itself may provide. The staff of these employers are not public servants and their pay and superannuation arrangements are not subject to the control of my Department.

Access for Home Helps in non-public service voluntary organisations to a pension scheme, similar to that provided for Home Helps in the employment of the HSE, is a matter, in the first instance, for the relevant employer. Pension access for Home Helps directly employed by the HSE was addressed by my Department in 2005. Access to the scheme is strictly limited to staff employed directly by the HSE. It is not available to Home Helps employed by other bodies, who are not public servants.

In June 2009, following a hearing of union claims for access to a pension scheme for staff of voluntary home help providers, the Labour Court recommended that this issue should be dealt with by way of a Gratuity Scheme based on 4.5 weeks’ pay per year of service, and that the scheme should take effect from the date of the Home Helps’ Collective Agreement of August 2000. The funding of such arrangements is not, per se, a matter for the HSE and, given the budgetary constraints under which the HSE must operate, the Executive is not in a position to provide financial assistance towards such arrangements. This position was advised to staff representatives and the home help providers in February 2012.

Hospice Services

The HSE Palliative Care Services Five Year/Medium Term Development Framework (2009-2013) details the required actions and initiatives necessary to address the gaps in service provision against the recommendations set out in the 2001 report. The HSE Assistant National Director for Acute and Palliative Care Services and the Clinical Palliative Care Programme Lead are working in partnership with regional services and national and local voluntary agencies to ensure the integration of service provision and that proposed service developments are in line with the priorities identified within the Framework.

Framework for Smaller Hospitals

146. Deputy Peadar Tóibín asked the Minister for Health the nature and extent of the consultation process on the framework for smaller hospitals which he published in May; and if he will make a statement on the matter. [30402/13]

I published the Framework for Smaller Hospitals and the Hospital Groups Report on 14 May 2013. In preparing the Hospital Groups Report, the project group carried out a comprehensive consultation process with all acute hospitals, including those hospitals covered in the Smaller Hospitals Framework, and other key advocacy groups which requested an opportunity to be consulted. In addition, a significant number of submissions were received by the project group.

Implementation of the recommendations of the Hospital Group’s Report has already commenced. I have personally visited the areas covered by each of the six groups with my Department and the Director General Designate of the HSE, explaining the policy objectives and listening to the questions the staff in each of those areas have about this policy implementation. Professor Higgins is also carrying out a debriefing exercise, supported by my Department and the HSE, to help ready the hospitals for the impending changes. Since the launch of the Report, he has held approx. fifty meetings with hospitals in this regard. The ongoing implementation of the Report will be overseen by a National Strategic Advisory Group, which will provide guidance and on-going direction to implementation of both the Hospital Groups Report and the Smaller Hospitals Framework and provide a forum to resolve issues and assist in the development of specific guidelines on the steps required for full implementation. Implementation will be driven by the HSE through the Hospital Groups Implementation team which is currently being established. As the new Hospital Groups are formed and their management teams appointed, each Group will give consideration to the consultation processes appropriate to that individual Group and to how stakeholders may be engaged with those processes.

The implementation of the Framework for Smaller Hospitals has largely been completed around the country and the objective is to complete the remaining necessary changes dependent on required capacity being available in the regions and hospital groups concerned. Hospital groups will be required to submit a strategic plan within one year of establishment which will outline its plans for future services within the group area, including those to be delivered in each smaller hospital. Smaller hospitals will play a crucial role in the delivery of services and when groups are established, services can be exchanged between sites. This will result in the enhancement of activity in smaller hospitals and will allow them to focus on the provision of care that is safe and appropriate.

Accident and Emergency Departments Closures

Deputy Sandra McLellan asked the Minister for Health if he will defer any decision on the cutting of accident and emergency services at Bantry General Hospital, County Cork, currently scheduled for 8 July, for a further 12 months until a full process of consultation with all relevant stakeholders, as outlined in the Future Care document, has been completed; and if he will make a statement on the matter. The Framework for Smaller Hospitals lists the services that are appropriate to a smaller hospital to ensure a safe, high quality services for patients. The reorganisation of services in Bantry General Hospital is being made in order to ensure that patient safety is not compromised and that the people of West Cork will get the best possible outcome from attending the appropriate hospital best suited to their health needs. Patients with minor injuries and low levels of complexity will continue to be treated in Bantry and patients with life threatening, traumatic and complex injuries will continue to go to Cork University Hospital, where they will have the best possible chance of recovery. These changes have been the subject of ongoing discussion with representatives of the wider West Cork community.

The present unit at the hospital treats people with minor injuries such as suspected fractures, sprains, minor cuts, burns and scalds 24 hours a day seven days a week. This service will continue at a Local Injury Unit at the hospital 12 hours a day, seven days a week from July of this year. The new service will have more experienced, highly qualified doctors with experience in emergency medicine and access to the support of consultants in emergency medicine who will visit on a frequent basis. Moreover, the designation of Bantry General Hospital as a remote rural hospital ensures that it may continue to provide acute hospital services required by the population of West Cork. Medical patients such as those suffering from blackouts, asthma and diabetes and medical emergencies such as acute cardiac problems and stroke will continue to be assessed and admitted to the hospital, as required, on a 24/7 basis.

Irish Statute Book Application

Deputy Andrew Doyle asked the Taoiseach if he will provide a detailed breakdown in the number of downloads that have occurred to date of the Irish Statute Book Application available for download on both the Apple AppStore and the Android Google Play Marketplace for smartphones and tablet devices; and if he will make a statement on the matter. The 'eISB app' facilitates access to the Irish Statute Book through mobile devices such as smart phones, tablets, PCs etc. The 'app' was commissioned by the Office of the Attorney General and released for Android platform devices in April 2011 and for iPhone/iPad devices in May 2011. The 'app' is free of charge and to date there have been 3798 downloads of the iPhone/iPad version and between 1000 and 5000 downloads of the Android version (Google Play Marketplace only provides a range for its downloads rather than a specific number).

The eISB has proved to be a popular and useful source of information. For instance, during the period January to March of this year, the eISB averaged almost 200,000 visits accessing approx 1,000,000 pages of information in each month. In total, there were over 2 million visits to the eISB web site in 2012 and this figure is likely to be exceeded this year.

State Solicitors

Deputy Andrew Doyle asked the Taoiseach the current number of State solicitors operating here in different geographical areas; if he will detail the number of and identify the locations in which State solicitors were appointed on indefinite contracts and-or contracts that do not need to be re-advertised before the practice of lifetime appointments was ended; the number and detail of the location of State solicitors who currently operate that work under fixed term contracts that need to reapply for appointment at the end of their term; and if he will make a statement on the matter. The appointment of State Solicitors was transferred from the Attorney General to the Director of Public Prosecutions in May 2007. When vacancies arise for State Solicitor positions the position is advertised and an open competition is run by the Office of the Director of Public Prosecutions.

There are 32 State Solicitor areas. Generally one State Solicitor is appointed to every county outside of Dublin. Exceptions to this rule apply in Counties Cork, Galway, Kildare, Limerick and Tipperary. In County Cork four State Solicitors have been appointed; one each for Cork City, Cork North East, Cork South East and Cork West. In County Limerick two State Solicitors have been appointed; one for Limerick City and one for Limerick County. Counties Galway, Kildare and Tipperary have all been divided into two State Solicitor areas.

The following table shows a list of all State Solicitor areas. The first column shows all areas where the State Solicitor holds a contract to age 65. The term of office of these State Solicitors may be extended by the Director of Public Prosecutions for one or more periods, each period not exceeding one year, subject to a maximum retirement age of 70 years. As these positions become vacant the practice is that they are advertised as fixed term contracts. The second column shows a list of all State Solicitor areas where the State Solicitor holds a fixed term contract.

156. Deputy Pat Deering asked the Tánaiste and Minister for Foreign Affairs and Trade if a request has been made to send Irish troops to Syria. [30100/13]

There are currently two United Nations peacekeeping missions which operate in Syria, the United Nations Disengagement Observer Force (UNDOF) and the United Nations Truce Supervision Organisation (UNTSO), the latter of which is headquartered in Jerusalem.

At the beginning of June a decision was taken by the Austrian Government to withdraw its peacekeeping contingent from UNDOF. In response to this, the United Nations approached a number of troop contributing countries, including Ireland, to discuss the provision of peacekeepers to replace the Austrian contingent and to bring the number of peacekeepers deployed to UNDOF to the full capacity provided for under its mandate. This request is currently under consideration.

Ireland has responded positively to a request received from the United Nations in April to deploy personnel to the headquarters of UNDOF. Three members of the Permanent Defence Force were recently deployed to the mission and a fourth member will be deployed to UNDOF in the coming weeks.

A further request was received in April seeking the deployment of additional UN Military Observers to UNTSO as part of the Observer Group which comes under the operational command of UNDOF. Twelve members of the Permanent Defence Force are currently serving with UNTSO. The request for additional observers is currently receiving consideration and a decision is expected shortly.

UN Secretary-General Ban Ki-moon last week announced the appointment of Major General Michael Finn, as Chief of Staff of UNTSO.

Deputy Andrew Doyle asked the Tánaiste and Minister for Foreign Affairs and Trade if annual reports are still made and compiled by the Permanent Mission of Ireland to the United Nations in New York; the use and purpose of these reports and who they are accountable to in his Department in Dublin; if these UN reports by the delegation are made public and if they are laid before Dáil Éireann and the Houses of the Oireachtas, either on an annual basis or otherwise; and if he will make a statement on the matter. It is not the practice for my Department’s overseas missions to prepare annual reports. In common with other missions, the Permanent Mission of Ireland to the United Nations reports on a regular and ongoing basis on a range of issues to relevant Divisions of my Department. Overseas missions also prepare annual business plans in consultation with and under the oversight of Headquarters. These plans are reviewed by the Management Advisory Committee of my Department. My Department is currently preparing an annual report which will be laid before the Houses of the Oireachtas, following approval by Government.

Overseas Development Aid Issues

Since the Millennium Development Goals were agreed by the global community in 2000, the number of people living in extreme poverty has fallen significantly – including in sub Saharan Africa where rates were highest. Since this time many more children have access to safe water; more girls and boys are attending school, and far fewer people are dying of preventable illness such as malaria and HIV. Much of this progress is attributed to the positive impact of international development assistance.

The Government, through the Irish Aid programme, has contributed to these achievements. A report produced earlier this month by the OECD, confirmed that over 80% of Ireland’s official development assistance is allocated to Least Developed Countries. This is the highest allocation from any OECD Member State and is an enormously positive reflection on our aid programme. These funds are directed at responding to emergency needs including food distribution, to supporting the development of social infrastructure in sectors such as education and health and to funding multi-sectoral programmes in social protection and nutrition.

Combating global hunger and under-nutrition is a key pillar of Ireland’s overseas development assistance programme. In 2008, the Hunger Task Force called on the Government to work towards an indicative target of directing twenty per cent of our overseas development assistance budget to hunger-related expenditure. In 2012 we delivered on that target.

In 2012, Ireland contributed €629 million to Official Development Assistance (ODA). Over 80% of this funding was managed under Vote 27 - International Cooperation through the Development Cooperation Division of my Department – known to the public as Irish Aid. The remainder consists of ODA eligible contributions from other Government Departments - principally the Departments of Finance, and Agriculture Food and the Marine - and Ireland’s share of the EU Development Cooperation Budget for 2012.

A full and detailed breakdown of Ireland’s ODA spending and of the specific sectoral allocations is presented in the Irish Aid Annual Report which is compiled by my Department, each year. The work on the report for 2012 is currently nearing completion. Preliminary expenditure data for 2012 demonstrates that approximately two thirds of Irish Aid funding was directed to the key sectors of Agriculture, Health, Education, Governance and Social Infrastructure Support. The remainder is allocated for Emergency, Humanitarian Assistance, (including food and non-food distribution), Water and Sanitation, Sectoral Support and Programme Management. I will arrange for a copy of the 2012 Irish Aid Annual Report to be forwarded to the Deputy following publication.
