Deputy Jackie Cahill asked the Minister for Health if the HSE will be requested to amend the file of a person (details supplied) to reflect an illness diagnosed in 1994 and 1995 in view of the fact that his or her file does not include same; and if he will make a statement on the matter. Amharc ar fhreagraThe recording of a diagnosis, and other clinical details on a patient's healthcare record, is an issue between the patient and his clinician and not an issue in which I can have any involvement. It is my understanding that if a patient wishes to have their clinical record amended, the patient may make such a request to the individual hospital concerned and/or the HSE Data Protection Office.
