asked the Minister for Health if he will issue a medical card to a person (details supplied) in Cork, suffering from spastic colon whose wages are above the limit and whose medical bill is a minimum of £40 per month for drugs and £18 per month for doctors' fees.

The determination of the eligibility of an applicant for a medical card and the issue of a card in appropriate cases is a matter for the chief executive officer of the relevant health board.

It is understood that the individual in question called to the offices of the Southern Health Board some time ago where he was requested to fill up an application form for a medical card and to have a certificate of income completed. However, the board did not subsequently receive the completed forms. Arrangements have been made, therefore, to have a fresh application form and certificate of income forwarded to the applicant. If he will return the completed forms to the Southern Health Board, his application will be considered.
