asked the Minister for Social Welfare the reason no application forms for the family income supplement were available in social welfare offices, post offices, or health centres in Limerick city during the week beginning 26 November 1984.

A supply of application forms and explanatory leaflets was issued to all employment exchanges on 1 October 1984 and further supplies were issued to local offices including Limerick Employment Exchange on request. Application forms were, therefore, available to the latter office as required. Forms were not distributed to post offices or health centres having regard to the extensive advertising of the scheme which included a coupon for requesting an application form.

asked the Minister for Social Welfare the total number of applications received for the family income supplement by (a) 31 October and (b) 30 November.

Final figures are not yet available as to the number of applications for family income supplement made to the Department up to the end of November 1984. Some 600 were received before the end of October 1984.
