Nursing Homes Support Scheme Eligibility

540. Deputy John Paul Phelan asked the Minister for Health if there is a legislative barrier preventing the temporary postponement of the taking up of the fair deal package by those who have successfully completed the application process but whose medical condition may have improved enough to allow them to live at home longer than previously anticipated, without them having to restart the process in its entirety again; and if he will make a statement on the matter. There is no legislative barrier preventing an applicant from postponing a place in a nursing home under the Nursing Homes Support Scheme Act 2009. However, the HSE does try to accommodate, as far as possible, applicants who make an effort to remain at home rather than go into long-term residential care. Generally a funding approval will be kept open for 6 months provided contact with the local Nursing Homes Support Office is maintained throughout this period.

National Children's Hospital Status

The National Paediatric Hospital Development Board is the statutory body responsible for planning, designing, building and equipping the new children's hospital. The new hospital will be co-located with St James's Hospital, and ultimately tri-located with a maternity hospital to be developed on campus. In addition to the main hospital, the project includes two satellite centres at the campuses of Tallaght and Connolly Hospitals.

The Project Brief has now been completed and approved. The Project Brief sets out details of the specialties to be provided and the planned accommodation. This will include 384 in-patient beds, all in single en-suite rooms with in-room parent accommodation, a further 85 day-care beds and 14 theatres, including three hybrid theatres to facilitate access to imaging during surgery, all in the main hospital. There will be 111 outpatient consulting examination rooms across both the main hospital and the two satellite centres, as well as ED and urgent care facilities.

A design team has been procured and is working on detailed design development, with the aim of submitting a planning application in June 2015. Subject to planning, work will commence at the main site at St. James's, and at satellite centre sites at Connolly and Tallaght, in January 2016.

The Health Information and Quality Authority (HIQA) began to regulate residential services for people with disabilities on 1 November 2013, pursuant to Statutory Instruments 366 (2013) and Statutory Instrument 367 (2013), which assign responsibility to HIQA for registering and inspecting residential services for children and adults with disabilities, including respite services.

My Department has been informed by HIQA that there are currently 88 registered service providers for Designated Centres for Persons (Children and Adults) with Disabilities, regulated under the Health Act 2007. This is not the total number of designated centres, there are many providers who have multiple designated centres.

The information requested by the Deputy in the second paragraph is not readily available but I will arrange for it to be forwarded to the Deputy as soon as it has been collated.

Mental Health Services Provision

The Health Service Executive operates the General Medical Services scheme, which includes medical cards and GP visit cards, under the Health Act 1970, as amended. It has established a dedicated contact service for members of the Oireachtas specifically for queries relating to medical cards and GP visit cards, which the Deputy may wish to use for an earlier response. Contact information has issued to Oireachtas members.

Deputy Caoimhghín Ó Caoláin asked the Minister for Health the position regarding the National Centre for Medical Genetics; the reason it is no longer to be a national centre; the plans in place to have a national centre; if it is envisaged that adult services and external clinics will be dropped from this; the place the centre is to have in the Irish national plan for rare diseases in view of the fact that the work of geneticists can ultimately save money with preventative care and the fact that Ireland has a low number of geneticists per capita when compared to other jurisdictions; and if he will make a statement on the matter. The service for medical genetics in Our Lady’s Children’s Hospital, Crumlin (OLCHC) provides services for patients and families in Ireland affected by, or at risk of, a genetic disorder. It comprises of three integrated units: a clinical genetics service; a cytogenetics laboratory and a molecular genetics laboratory. The service is under the governance of OLCHC, which has recently made a decision to discontinue the separate identification of the service as the National Centre for Medical Genetics, in the interest of clarity on governance arrangements. I am assured that there has been no reduction in the scope of the services provided.

There have been rapid and progressive developments in genetics and genomic medicine over the past five years, and it is well recognised as one area of medicine that will continue to rapidly develop over coming years. A review of the service at Crumlin, commissioned by the HSE was submitted in May this year. This report made a number of recommendations on both internal and national service issues, which will be addressed by the HSE in conjunction with OLCHC / the Children's Hospital Group. The report included a recommendation that a working group should be established, to develop a National Genetic and Genomic Medicine Network that reflects best international practice. Any concerns on the network of services will be addressed by this group's work.

The National Rare Disease Plan recognised that the timely diagnosis of rare diseases is essential. Effective genetic services have a role to play in diagnosing rare diseases; and it was in this context that the plan referred to the work of the NCMG at Our Lady's Children's Hospital, Crumlin. A National Clinical Programme for Rare Diseases has been established in the HSE. A Clinical Lead for this programme has also been appointed. This clinical programme will ultimately be a channel for advancing and implementing recommendations in the plan, such as those on National Centres of Expertise for rare diseases.

An Injury Grant Scheme is provided for under Section 12 of the HSE Employee Superannuation Scheme 2010 (SI 362/2010). The scheme provides for the granting of an allowance to an officer who is injured in the discharge of their duty, specifying that the allowance shall not exceed 5/6ths of their remuneration.

Any social welfare benefits, which are payable to the employee, are subtracted from this grant payment and the net sum is then multiplied by the person’s degree of impairment to calculate the injury allowance. This results in different rates of injury grant being paid to the employee based on the seriousness of their injury. The allowance can subsequently be revised in light of changes in level of impairment.

Health Screening Programmes

Deputy Helen McEntee asked the Minister for Health his views on extending the free cervical smear testing programme to include under 25 year olds; if the scheme will be extended on the basis that should a person under 25 years decide they require a smear test, based on family history, if they will then be included; the amount it would it cost to include under 25 year olds in this programme; if he will provide the figures for the number of women under 25 years of age who die from cervical cancer each year. CervicalCheck, the National Cervical Screening Programme, was introduced in 2008 and offers free smear tests to women aged 25-60. International best practice recommends that a population based cervical screening programme should target women aged from 25 or 30 years to 60 or 65 years.

Invasive cervical cancer is extremely rare in women aged under 25, with less than 5 women in this age group being diagnosed with this condition each year. There was one death from cervical cancer in women below the age of 25 in the years 2008-2012. While changes in the cells of the cervix are very common for those under 25, in the vast majority of cases these changes will settle and return to normal without intervention. If the screening age was lowered to invite those under 25 for smear tests, young women could be subjected to unnecessary treatment that could have potential long-term negative effects on their health and fertility.

I would encourage all women aged under 25 to be vigilant about their health and to consult their GP immediately if they have any symptoms which would raise the suspicion of cervical cancer. If necessary, their GP will refer them to a gynaecology clinic for further investigation. Meanwhile, any women concerned about their family risk of cancer should discuss this with their GP.

I would also encourage all 12-18 year old schoolgirls to avail of the free HPV vaccination which is offered in second level schools.
