Deputy Clare Daly asked the Minister for Health given the unprecedented suspension of a midwife (details supplied) on 12 September 2014 pending the outcome of a systems analysis investigation by the Health Service Executive, which he repeatedly claimed would be produced by mid-March 2015, the reason for the failure by the executive to date to produce a final report on the investigation; and when this will be completed.
