117. Deputy James Lawless asked the Minister for Health if he will examine the composition of the current internal hospital review teams conducting internal investigations into all serious incidents in hospitals with particular reference to the lack of external independent persons being members of these review teams. As this is a service matter, I have asked the HSE to respond to you directly. If you have not received a reply from the HSE within 15 working days please contact my Private Office and my officials will follow the matter up.

For the Deputy's information, the HSE's Serious Incident Management Policy (SIMP) outlines the processes for conducting investigations and properly allows this work to progress without Departmental involvement.

In addition, the Health Information and Quality Authority (HIQA) is currently developing Standards to support the management of Patient Safety incidents in collaboration with the Mental Health Commission.
