I wish to draw the Deputy’s attention to replies to Parliamentary Questions 402 of 16 June 2015, 288 of 8 December 2015, 333 of 7 July 2016 and 674 of 13 December 2016. In each of these replies, an Irish Air Corps Safety Management Systems Review was inadvertently included as a published report. This in fact was not the case; rather, it was completed in November 2013 and as a review completed for internal use, it was not prepared for publication.
