View answerMy officials in the Department of Health have engaged with the HSE on behalf of the family to determine how the recommendations in this report were communicated nationally. The HSE has advised that the report, findings and recommendations were discussed at a National Performance Meeting with the National Acute Directorate that had a specific focus on quality and patient safety in early 2019.
