asked the Minister for Health if he is aware that when an insured person makes an appeal for a revised assessment under section 15 of the Health Act, 1953, he is obliged to present a health authority document to his employer; and that this form is framed in a manner which conveys to the employer and to the employer's office staff that the employee is an applicant for health services; and if he will direct health authorities to modify their verification forms.

While I am not aware that the practice to which the Deputy refers applies generally, I propose to take up the matter with health authorities in order to ensure that an unobjectionable procedure is followed in all cases.
