109 Mr. G. Mitchell asked the Minister for Public Enterprise if she will review the case of a person (details supplied) in Dublin 12 who is a Telecom Éireann pensioner and who applied for shares but was denied these as the share information office at her Department ruled that the application was incomplete due to the fact the mandate was not properly filled out. [19214/99]

In the case of this person the direct debit mandate in the application was incomplete and therefore it could not be processed. In accordance with procedures put in place following the closing of the retail offer for applicants who had not completed their direct debit forms, best efforts within the time available were made by the share information office to make a telephone call to the contact telephone number, if any, provided to obtain the bank account and/or sort code. In the event that the call was unanswered or such details were not provided during the call, the application could not be processed.
