I can confirm that my department received an application for disability allowance from this man on 12 October 2016.

When the application was examined by a deciding officer (DO), it was found that the application form was partially incomplete. The person concerned recently has been requested to complete the form as required by the DO to make a decision on his eligibility. On receipt of this information a decision will be made and the person concerned will be notified of the outcome.

I trust this clarifies the matter for the Deputy.
