Under the Health Act 2004, the Health Service Executive (HSE) is required to manage and deliver, or arrange to be delivered on its behalf, health and personal social services. Section 6 of the HSE Governance Act 2013 bars the Minister for Health from directing the HSE to provide a treatment or a personal service to any individual or to confer eligibility on any individual.

The Health Service Executive (HSE) has statutory responsibility for decisions on pricing and reimbursement under the community schemes, in accordance with the Health (Pricing and Supply of Medical Goods) Act 2013. In making a relevant reimbursement decision, the HSE is required under the Act to have regard to a number of criteria including clinical efficacy, the health needs of the public, cost effectiveness and potential or actual budget impact.

Currently, Consultant Endocrinologists may apply to the HSE, on behalf of specific patients with type 1 diabetes, for reimbursement support of FreeStyle Libre glucose monitoring sensors. The application process is undertaken by means of a dedicated online portal, which has been operational since 3 April 2018. In line with the recommendations of the Health Technology Assessment Group, access to this product was made available to children and young adults, aged between 4 and 21 years of age. However, the online application process does cater for the Consultant to make an application in very exceptional circumstances for a type 1 diabetic patient who is over 21 years of age.

Other continuous glucose monitoring systems (e.g., Dexcom, Medtronic) are available for type 1 diabetic patients.

The Health Information and Quality Authority recently completed a Health Technology Assessment (HTA) in respect of the provision of glucose monitoring systems to adults with Type I diabetes in Ireland. This was published on 29 September 2023 and is available at www.hiqa.ie/reports-and-publications/health-technology-assessment/rapid-health-technology-assessment-continuous. The HTA advises the HSE that a single managed access programme with clearly defined eligibility criteria would improve equity of access and could help to control costs.

Officials in my Department engage with the HSE Diabetes clinical team in consideration of the future development of diabetes related services.

Cross-Border Co-operation

Enhancing North-South cooperation is a priority for the Irish Government. I am committed to working with partners in Northern Ireland to improve cooperation in health and social care where there are clear benefits for both jurisdictions and for patients across the island.

There are a number of agreements in place covering specific specialist Cross Border initiatives that ensure that patients can receive a range of medical procedures/services as close to home as possible. This has helped reduce travel time, and the ease of access, which has been hugely beneficial for patients, their families and carers.

The North West Cancer Centre at Altnagelvin Area Hospital is a £66m sterling capital development project that has been funded on a North-South basis. This Centre, incorporating the new Radiotherapy Unit, commenced services at end 2016. In 2021, 253 Irish patients were treated in Altnagelvin, with 267 Irish patients treated in 2022 and 213 patients referred YTD September 2023. Since it opened, over 1,220 Irish patients have been treated at the Centre.

The North West Cardiology Primary Percutaneous Coronary Intervention (PCI) service agreement, which has been in place since 2016, provides for the provision of Primary PCI to Donegal patients at Altnagelvin Hospital. The cardiology service is provided to patients with a diagnosed ST-Elevation Myocardial Infarction condition (i.e., a heart attack). The figure for patients transferred for PCI services from Letterkenny University Hospital (LUH) in 2022 was 72, and there were 118 transferred for PCI services from LUH YTD July 2023.

Furthermore, there is ongoing and good engagement between officials in my Department and their counterparts in the Department of Health, Northern Ireland. Most recently, Senior Officials from both Departments met in Dublin in July, n follow-up to a previous engagement in Belfast in January, to discuss areas of health cooperation which could be expanded to benefit residents on the island of Ireland.

This year, my Department also established an intra-departmental North-South Working Group to support this broad North-South cooperation in all areas of my Department's work. The Group was set up with Secretary General Robert Watt's endorsement to underline the importance of this work. The Group has identified a number of areas which would benefit from enhanced North-South cooperation which is guiding Senior Officials’ engagement. I am committed to continuing close and productive cooperation with Northern Ireland.

Deputy Neasa Hourigan asked the Minister for Health further to Parliamentary Question Nos. 604 of 25 April and 468 of 18 May and 702 of 27 June 2023, the number of people with severe and enduring mental health difficulties under the care of the Cobh/Glenville and Midleton/Youghal adult mental health teams who have been transferred to continuing care or high-support hostels outside those HSE catchments since February 2021 when the Owenacurra Centre took its last referral for residential placement; the number of people in this cohort of service-users within those HSE catchments who have been transferred in the same timeframe to nursing homes, either within those HSE catchments or outside them; the reason that this information was not provided in the response to the Parliamentary Question, given that the information could be obtained by requesting it from the consultant psychiatrists on the two mental health teams under whose care the service-users in questions would have been, and given that similar information on the transfer of east Cork service-users was provided in response to previous Parliamentary Questions; and if he will make a statement on the matter. Covid-19 Pandemic Supports

Deputy Neasa Hourigan asked the Minister for Health his views on the planned ending of a special Covid leave with pay arrangement for healthcare workers on 31 October 2023 with the chronic and debilitating nature of that condition for this cohort of staff, who were previously given assurances by his Department that their needs would be met and that these healthcare workers contracted Covid in high-risk settings in circumstances that amount to being considered a workplace injury in other EU Member States; and if he will make a statement on the matter. For a certain cohort of employees suffering from long-Covid, a Special Scheme of Paid Leave for Employees Unfit for Work Post Covid-19 Infection, specific to the public health service was introduced on 1 July 2022. Prior to this date, this cohort of employees had been supported financially by Special Leave with Pay that was introduced by the Department of Public Expenditure, National Development Plan and Reform (DPENDPR) at the start of the pandemic.This Special Scheme was due to conclude on 30th June this year. In recognition of this ongoing matter, sanction to extend the Special Scheme for a period of 12 months was sought from the DPENDPR, however the sanction provided was for a period of just four months to 31st October 2023.As my wish is to continue to support this cohort of health care workers, my Department sought approval from the DPENDPR to further extend the Special Scheme to end June 2024 in line with the original request to extend. Approval has now been received from DPENDPR to extend the Scheme to 31st March 2024, after which date, any employee on the Special Scheme remaining unwell, may utilise the provisions of the Public Service Sick Leave Scheme thereafter.

In respect of the EU recommendation regarding the recognition of Covid-19 as an occupational illness/injury/accident, that is a matter for my colleague Minister Humphreys in respect of the Social Welfare Consolidation Act, however it should be noted that the EU recommendation is in respect of Covid-19 and not long Covid, and Member States vary hugely in respect of the treatment of the recognition of the recommendation.
