asked the Minister for Social Welfare if a death grant is payable to a person (details supplied) in Dublin, in respect of a relative who died in St. Phelim's Hospital, Cavan recently, if she as the surviving next-of-kin is entitled to the payment of such grant; and, if so, if he will make the necessary arrangements relating to the sending out of the appropriate application forms.

Entitlement to a death grant by the person referred to by the Deputy cannot be determined until the appropriate claim form has been completed and returned to my Department. A claim form cannot be issued by post immediately due to the postal dispute. The form may, however, be obtained at my Department's Public Office, Oisin House, 212/213 Pearse Street, Dublin 2.
