Written Answers. - Birth Certificate Form.

asked the Minister for Health the reason each of the items listed on the form A requisition for a certificate of birth is sought.

Form A, Requisition for a Certificate of Birth, is a form which has been designed to facilitate applications for birth certificate at a reduced fee.

The items of information requested on the form and the reasons why they are sought are as follows:

Name of the Act under which the certificate is required—this is required to establish the authority under which the applicant is entitled to a certificate at a reduced fee.

Name of person, date of birth, place of birth, father's name, father's occupation, mother's full name, mother's maiden surname—these are required to enable the entry of birth to be located in the register of births and to confirm that it is the correct entry.

Signature and address of applicant—these are required to authen-ticate the application and to enable the certificate to be posted to the applicant.

Date of application—this enables applications to be dealt with and filed in sequence.
