I have been advised by the Revenue Commissioners that the completed form S.L.4 was issued by the inspector of taxes on 10 November 1983, to the solicitor acting for the taxpayer. The delay in issuing the form arose because insufficient details had been supplied with the initial application made on 14 August 1983. The full information required by the inspector was not supplied until 4 November.
