Written Answers. - Notification of Risk Forms.

199 Mr. Yates asked the Minister for Health the reason forms are issued to patients in relation to notification of risk in certain hospitals; the purpose of such forms and the meaning of the particular reference to lack of facilities in a form provided by a hospital (details supplied) in Cork; and if he will make a statement on the matter.

I have had inquiries made of the Southern Health Board and I understand that the form referred to by the Deputy is of a general type which is recommended by medical insurance authorities for use by consultant medical staff where they may have some dissatisfaction with patient facilities available to them. The form was not for patient use. The Southern Health Board has indicated that this form was in use in the unit in question for a short period of time and was in fact withdrawn from use about 12 months ago. The Southern Health Board has further indicated that the patient facilities in the unit are satisfactory.
