310 Mr. Haughey asked the Minister for the Environment and Local Government if he will forward to Deputy Haughey an application form for a certificate of fitness together with a specimen certificate of fitness with regard to drivers as promised in the reply to Parliamentary Question No. 670 of 27 January 1999; if such documentation exists; and if he will make a statement on the matter. No formal application form for a certificate of fitness exists as such certificates are requested either by the courts or by driver testers who may defer a decision on a driving test until such a certificate is submitted. I have arranged for a further specimen copy of the required medical report and of the certificate of fitness to be forwarded to the Deputy.
