321 Mr. Allen asked the Minister for Health and Children his plans to introduce a form of hospital practice in order to change the methods of reporting medical errors. [27751/01]

The introduction of comprehensive clinical incident reporting will require a major change in attitudes by clinicians and those with responsibility for managing services. Experience elsewhere suggests that an open, non-punitive approach to incident and error reporting is a fundamental requirement for the successful introduction of such systems.
