359 Mr. Neville asked the Minister for Health and Children further to Parliamentary Question No. 303 of 20 May 2003, if he will outline the full details of the report from the Limerick mental health services in the case of a person (details supplied) who died by suicide in the case of the health board. [16770/03]

A report by the Limerick mental health services on the circumstances of the death of the person concerned has been received by my Department from the Mid-Western Health Board. I am informed that the Mid-Western Health Board has now decided to establish an independent clinical review and audit regarding the care and treatment of the person concerned. It would be inappropriate for me to comment further pending the completion of the independent review.
