495 Deputy Clare Daly asked the Minister for Health the reason the parental consent form (details supplied) for dental service in the Cavan-Monaghan area accepts the consent of one parent only and does not adhere to the staff guidelines for obtaining consent for non-emergency treatment services from parents of children and young people under the age of 18 years.
