604. Deputy Billy Kelleher asked the Minister for Health the community health care organisations which have appointed a head of social care, in tabular form. [23935/16]

As the Deputy's question relates to service matters, I have arranged for the question to be referred to the Health Service Executive (HSE) for direct reply to the Deputy.
