The Department of Arts, Heritage, Regional, Rural and Gaeltacht Affairs was established under that title on 7 July 2016. It has responsibility for all the matters that previously came within the remit of the Department of Arts, Heritage and the Gaeltacht, which was itself established on 2 June 2011, and in addition is also responsible for certain functions transferred from the Department of the Environment, Community and Local Government and from the Department of Communications, Energy and Natural Resources. Accordingly, the information requested by the Deputy is set out in the tables below in respect of the period from 2 June 2011, except in the case of those functions that transferred in 2016. No expenditure was incurred in relation to interview training and preparation.

In light of the large volume of information requested by the Deputy, arrangements are being made for the information requested in respect of those agencies under the aegis of my Department to be collated and forwarded to the Deputy in line with the timelines set out in Standing Orders.

I propose to take Questions Nos. 167 to 169, inclusive, together.

The Central Applications Office (CAO) processes all applications for nursing and midwifery undergraduate courses in Irish Higher Education Institutions (HEIs). Decisions on admissions to undergraduate courses are made by the HEIs who instruct the CAO to make offers to candidates who meet the required criteria.

I can confirm that the places for undergraduate degree programme are currently set at 1,570. This figure was considered to ensure that capacity factors including clinical placement requirements are available to each student on the programme. Each placement requires supervisory, teaching and assessment support, all of which are resource intense. It should also be noted that during the final year of the programme the student is paid 70% of the first point of the staff nurse salary scale for a period of 36 weeks.

Allocation of these places is made to both standard and mature applicants. Standard applicants include those applying on the basis of leaving certificate results and QQI FET (FETAC) level 5 awards. Mature applicants apply on the basis of being over 23 years and an assessment test. Approximately 15% of places are allocated to mature applicants.

It is the decision of the universities and colleges as to the quota of places made available to QQI FET applicants and not all courses will accept these applicants. The overall quotas available for QQI FET applicants is limited - for example in 2016, there were 1,581 applications using QQI FET awards for approximately 98 places.

I would also like to advise the Deputy that the budget for these degree programmes is held by the Department of Education and Skills.

During 2016, the Department wrote to the Minister of Education and Skills highlighting the current shortage of nurses in Ireland, especially psychiatric nurses. The Department sought an increase in the number of undergraduate nursing places available in HEIs in the 2016/2017 academic year in order to reduce the shortage of psychiatric nurses in Ireland.

In that regard, I wish to advise you that the Department of Education and Skills and the Higher Education Authority agreed to increase the Psychiatric nursing places by 60 for the 2016/2017 intake. Workforce planning for replacement psychiatric nurses that are due to retire in the next few years is also currently under review.

The HSE has statutory responsibility for decisions on pricing and reimbursement of medicines under the community drugs schemes, in accordance with the Health (Pricing and Supply of Medical Goods) Act 2013.

In reaching its decision, the HSE examines all the evidence which may be relevant in its view for the decision (including the information /dossier submitted by the Company) and will take into account such expert opinions and recommendations which may have been sought by the HSE at its sole discretion (for example, from the National Centre for Pharmacoeconomics).

In considering an application, the HSE will also have regard to Part 1 and Part 3 of Schedule 3 of the 2013 Act. Part 3 requires the HSE to have regard to the following criteria:

1. the health needs of the public;

2. the cost-effectiveness of meeting health needs by supplying the item concerned rather than providing other health services;

3. the availability and suitability of items for supply or reimbursement;

4. the proposed costs, benefits and risks of the item or listed item relative to therapeutically similar items or listed items provided in other health service settings and the level of certainty in relation to the evidence of those costs, benefits and risks;

5. the potential or actual budget impact of the item or listed item;

6. the clinical need for the item or listed item;

7. the appropriate level of clinical supervision required in relation to the item to ensure patient safety;

8. the efficacy (performance in trial), effectiveness (performance in real situations) and added therapeutic benefit against existing standards of treatment (how much better it treats a condition than existing therapies);

9. and the resources available to the HSE.

I am informed that, following a request from the HSE, the NCPE carried out an assessment of the manufacturer's economic dossier submitted in March 2016 on the cost effectiveness of Orkambi. This dossier included details on all relevant costs and relevant cost offsets including those associated with hospitalisation, disease management costs, intravenous antibiotics, adverse events and any additional costs arising in patients not taking Orkambi.

The NCPE has completed its Health Technology Assessment and this is available on its website. It was submitted to the HSE in June 2016. The NCPE determined, following an evaluation of the economic dossier, that the manufacturer failed to demonstrate cost-effectiveness or value for money from using the drug. The NCPE have confirmed that all relevant costs were included in the analysis. In line with the HSE's assessment process, the HSE Drugs Committee considered the NCPE recommendation, the manufacturers submissions and commercial offer and other information. Following this consideration the Drugs Committee did not recommend reimbursement at the current price. The HSE Directorate considered the Drugs Committee’s recommendation in December and the Directorate took the decision not to reimburse at the current price offered by the company.

The HSE has since re-entered into negotiations with Vertex, the manufacturer of Orkambi, with a view to significantly reducing the cost of Orkambi. A meeting was held in December and again in early January. Following completion of the negotiation process, I am advised that the HSE’s Drugs Committee met at the end of January and considered the manufacturer's latest price offerings. The matter is currently under consideration by HSE Leadership. A decision on reimbursement will be made on objective, scientific and economic grounds in line with the 2013 Act.

Medicinal Products Reimbursement

I am informed that the HSE received a request and economic dossier from the manufacturer of Respreeza for maintenance treatment of emphysema in adults with documented severe alpha1-proteinase inhibitor deficiency.

In June of this year the HSE asked the NCPE to carry out a health technology assessment on the cost effectiveness of this treatment. The NCPE completed its assessment and made a recommendation on 9 December 2016. The NCPE determined that the manufacturer failed to demonstrate cost-effectiveness of the drug and did not recommend it for reimbursement.

The HSE will consider the NCPE assessment, and other expert advice, as part of its decision-making process for reimbursement. This decision will be made on objective, scientific and economic grounds by the HSE in line with the 2013 Act.

The operation of compassionate access schemes is at the discretion of manufacturers. There is no provision in Irish legislation for the approval of compassionate use programmes for specific groups of patients with an unmet medical need. I have previously asked manufacturers to show compassion and to maintain compassionate access schemes for patients during the assessment process by the HSE and this remains the position. The manufacturer of Respreeza, CSL Behring, has confirmed that the compassionate programme has been extended until the end of February 2017.
