459. Deputy Pearse Doherty asked the Minister for Health the individual funding that was allocated to a training centre for adults with disabilities in County Donegal for the provision of services, training and supports which was to be provided to a person (details supplied); the details of the services, training and supports provided to the person in addition to the costs associated with each and which was incurred through the provision of same for each of the years 2015, 2016 and to date in 2017; and if he will make a statement on the matter. Deputy Pearse Doherty asked the Minister for Health the reason a transitional care plan compiled in respect of a person (details supplied) outlining the individual care needs of that person was not implemented by a training centre for adults with disabilities in County Donegal; if an individualised care plan was ever created for the person while the person was a service user at the aforementioned facility; if so, if he will make this plan available to the person for the person's information; and if he will make a statement on the matter. Deputy Pearse Doherty asked the Minister for Health the reason formal written confirmation has never been issued to a person (details supplied) to indicate that the person's individual care needs cannot be accommodated through the various programmes offered by the National Learning Network; if written confirmation will now be issued to the person to confirm same; and if he will make a statement on the matter. The Government is committed to providing services and supports for people with disabilities which will empower them to live independent lives, provide greater independence in accessing the services they choose, and enhance their ability to tailor the supports required to meet their needs and plan their lives. This commitment is outlined in the Programme for Partnership Government, which is guided by two principles: equality of opportunity and improving the quality of life for people with disabilities.

As the Deputy's question relates to an individual case, I have arranged for the question to be referred to the Health Service Executive (HSE) for direct reply to the Deputy.
