Cannabis for Medicinal Use

My Department and HSE officials are currently considering funding and access proposals for cannabis-based products for medicinal use including work to establish a reliable and affordable quality controlled cannabis product supply for the Irish market.

In relation to the reimbursement of cannabis-based products in the community drug schemes, I understand that the HSE is currently considering a reimbursement application, in line with the statutory requirements for the schemes.

Work is under way on setting up a Centre of Excellence for Narcolepsy based in St. James’s Hospital for patients with all forms of narcolepsy in adults and in children transitioning from the paediatric services, as was committed to in the HSE 2018 National Service Plan. The level of funding for the infrastructure, staffing and recruitment required for this centre is under consideration by the HSE.

On 9 February 2018, an agreement was reached between my Department, the HSE and health sector Trade Unions in relation to a process aimed at resolving the pay restoration issue for staff employed by section 39 bodies. The HSE will carry out an analysis to establish, with supporting evidence, the following:

- Whether, when and to what extent reductions in pay rates were applied during the crisis in each relevant organisation;

- Whether, when and to what extent restoration of pay reductions has happened;

- Identify the financial implications for each organisation, taking account of all sources of funding, associated with addressing the issues identified and propose an appropriate plan for phased resolution in each case.

The HSE has issued detailed questionnaires to an agreed list of organisations.  The interim report, to which the question refers, is expected from the HSE by the end of March 2018.

The HSE has statutory responsibility for decisions on pricing and reimbursement of medicines under the community drug schemes, under the Health (Pricing and Supply of Medical Goods) Act 2013. In reaching a decision, the HSE examines all the evidence which may be relevant in its view for the decision and will take into account such expert opinions and recommendations which it may have sought, including, for example, advice from the National Centre for Pharmacoeconomics (NCPE). Sapropterin (Kuvan) was considered under the national pricing and reimbursement processes in 2009. At that time, insufficient evidence was available to support the pricing and reimbursement application submitted. In 2016, the HSE was advised that a new health technology assessment dossier would be submitted on sapropterin. The NCPE completed its assessment of the new dossier in September 2017 and did not recommend sapropterin for reimbursement, as it was not deemed cost effective at the price sought by the company. My Department was advised that Sapropterin was discussed at a recent HSE Drugs Group Meeting. The Drugs Group was not in a position to make a recommendation to HSE Leadership following that meeting. It has sought additional information and deliberations are ongoing.

I continue to support the HSE in its efforts to bring down the waiting times for and improve access to scoliosis services. The long-term strategy to develop sustainable scoliosis services from 2018 has been prioritised by the Department of Health and the HSE in the 2018 HSE National Service Plan.

€9 million funding has been provided in 2018 to improve access to paediatric orthopaedic services including scoliosis. The 2018 HSE National Service Plan includes a commitment from the HSE to continue to improve access to paediatric orthopaedics, including surgery for scoliosis, in conjunction with the Children’s Hospital Group to achieve clinically appropriate waiting times. This will be underpinned by the development of a standardised pathway of care for children and adolescents with scoliosis which will be evidence-based and patient-centred.

The HSE, in their Service Plan, has also committed to the development of a service for young adults with scoliosis in the Mater Misericordiae University Hospital, Dublin, and Cappagh Orthopaedic Hospital for patients transferring from paediatric services.

In addition, a Paediatric Scoliosis Services Co-Design group is in place, which includes clinicians, hospital staff and representatives from the scoliosis advocacy groups, to design a comprehensive contemporary and patient-centred approach to the delivery of scoliosis services.
