A claim to optical benefit was received from the person concerned on 16 September 1985. The claim was delayed because he omitted to state his RSI/insurance numbers on the application form. Further inquiries were necessary in order to establish his numbers and insurance record. These inquiries have now been completed and it has been established that he is qualified for benefit. His claim has been approved and the form of authorisation for presentation to his optician has been issued to him.
