607 Deputy Martin Ferris asked the Minister for Health the reason a person (details supplied) in County Limerick has not received a medical card after applying for it five months ago; the reason no effort was made by the client registration unit after this Deputy wrote to them on 23 January to inform the applicant as to whether their application was lost or being dealt with; and the reason no one has contacted the applicant to advise on the steps that must be taken to rectify the situation. As this is a service matter it has been referred to the Health Service Executive for direct reply to the Deputy.

608 Deputy Martin Ferris asked the Minister for Health the reason a person (details supplied) in County Limerick has not received a medical card which was put on hold on 2 December 2011 when the Health Service Executive requested further documentation which was furnished; the reason no action was taken to inform the applicant as to whether their application was lost or being dealt with and the reason no one has contacted the applicant who has had five operations since 2008 to advise them on when their medical card will be issued.
